24/7 access to care management is a standard. Chronic Disease Solutions decided to go beyond the requirement in order to better serve your patients. Patients have 24/7 access to our Care Coordinators who, when needed, can refer the call to one of our RNs or Physicians.
Health Coach
Chronic Disease Solutions is committed to a healthy lifestyle. We have a certified Health Coach on staff that our Care Coordinators refer patients to who creates a tailored health plan based on the patient’s needs.
Implementation
For zero costs our staff goes onsite for the implementation of CCM, which allows us to quickly ID workflows best suited for your needs, ID CCM participants, train your staff, etc. We utilize direct access to our clients EHR’s to gather the necessary information. We may also use CCD’s transferred through secure messaging
Enrollment Specialist
After implementation, Chronic Disease Solutions will place an Enrollment Specialist at your practice. The amount of time will be determined by you and our implementation team. We have found Enrollment Specialists greatly increase the rate of consents gathered as well as take strain off your staff!
